TO BE A TT ACHED AND MADE PART OF the specified contract by and between the State of Utah Division of Purchasing and
CenturyLink Public Communications, Inc. (Referred to as CONTRACTOR).
BOTH PARTIES AGREE TO AMEND THE CONTRACT AS FOLLOWS: Change the commission rate to be paid by
Contractor on all calls from 76.9% to 78.2% beginning with the December 2017 commission cycle, paid in January 2018. Other
than the commission rate, the remaining pricing structure as amended in Amendment 1 with an effective date of 3/21/2016 shall
remain the same.
Effective Date of Amendment: I 2/8/20 I 7
All other terms and conditions of the contract.• including those previously modified, shall remain in full force and effect.
